Step 2: Gather Required Information & Documents

  • End User & Authorized Contact Information

    Please provide the following exactly as it appears with your current provider:

    • Billing Address
    • End User Name (Business Name)
    • Authorized Person’s Name (the individual authorized to make changes to the account)
    • Billing Telephone Number associated with the account
  • Supporting Documents
    • Letter of Authorization (LOA) granting permission to transfer the number to Agent Legend.
